Section 2: Completing the Onboarding Process

Provide the Name and Address of the Controlling Entity.

If the Controlling Entity owns previously awarded 8(a) businesses, select Yes. If not, select No.

By selecting Yes, a dropdown will appear to add all 8(a) awarded businesses. Please ensure you add complete and accurate information before selecting Add to complete an entry. Additionally, please make sure to add all awarded businesses to ensure all applicant businesses are aligned in your Controlling Entity Dashboard once approved.

If the Controlling Entity has any 8(a) awarded firms that are owned by subsidiaries or holding companies, please select Yes, LLC; Yes, Corp; or Yes, Partnership based on the holding company’s organizational structure. If not applicable, select No.

Next, provide accurate and relevant information regarding the Controlling Entities:

• Tribe members

• Unemployment rate

• Per capita

• Poverty level percentage, and

• Access to capital markets

If the Controlling Entity is wholly or partially owned by other tribes, select Yes. If not, select No.

By selecting Yes, a dropdown will appear to add all tribal enterprises that have ownership. Please ensure you add complete and accurate information before selecting Add to complete an entry.

Finally, if the Controlling Entity owns or have subsidiaries that own 50% or more of any other companies aligned to the Controlling entity, select Yes. If not, select No.

By selecting Yes, a dropdown will appear to add all subsidiaries that meet the criteria in the question above. Please ensure you add complete and accurate information before selecting Add to complete an entry. Additionally, please make sure to add all subsidiaries to ensure the subsidiaries structure is accurate in your Controlling Entity dashboard, once approved.

Once complete with all questions in the questionnaire, select Next.

• Intended number of Native Hawaiians

• Unemployment rate of intended group

• Per capita of intended group

• Poverty level percentage of intended group

• Access to capital of intended group

If the Controlling Entity has any 8(a) awarded businesses that are owned by subsidiaries or holding companies, please select Yes, LLC; Yes, Corp; or Yes, Partnership based on the holding company’s organizational structure. If not, select No.

Next, provide a description of the activities the NHO has done so far to benefit Native Hawaiians.

Finally, if the Controlling Entity owns or have subsidiaries that own 50% or more of any other companies aligned to the Controlling entity, select Yes. If not, select No.

If the Controlling Entity POC navigates back to the Home Page while their onboarding application is still in draft state, in-progress application will appear at the top of the dashboard. Users may select Continue in the bottom right corner of the application section to resume their application. All other sections will be greyed out until the application is approved.

If an application has been submitted, the application section will show a Pending Approval tag in the top left. Users may view their completed application by selecting Open in the bottom right corner. All other sections will be greyed out until the application is approved.

If the user’s application is approved, the application section will disappear from the Home Page and all sections will be available for view and use.

If the user’s application is denied, the applications section will show a Rejected tag in the top left. All sections of the Home Page will remain greyed out.
